Report a Security Breach or Suspicious Incident

The Airline Pilots Security Alliance is responding to federal authorities’ requests for incident reports from flight and cabin crews, law enforcement and the public, involving suspicious persons or security lapses in-flight and in and near airports (i.e., multiple passengers standing all at once in the cabin when a pilot enters that part of the aircraft).  Information you provide will be reviewed by an APSA Security Specialist who is a former federal agent, de-identified, sensitive information removed, and then added to our Q/SAR database.  A Security Specialist may contact you for further information, only if you agree.

The deidentified Q/SAR Database will be continuously updated and available to any visitor at www.secure-skies.org.

After you submit your report, only the exact information you agree to will be forwarded to federal authorities, and then only with your specific permission. APSA DESTROYS ALL PERSONAL INFORMATION AFTER VERIFYING YOUR IDENTITY.
This information will greatly help the crews and the authorities visiting our site assess suspicious activities in a timely manner, and maintain continuous awareness of the level of threat.

The below form should be filled out in its entirety and then submitted to APSA.  Feel free to contact davidMackett@secure-skies.org regarding your submission, if you have any questions or concerns.
YOUR IDENTITY IS REQUIRED AND WILL BE VERIFIED.
